TFS returns exception during an API call when using: public TeamFoundationDataReader TrackMerges:

"TF246021: An error occurred while processing your request. Technical information (for administrator): SQL Server Error: 2627"

Windows event log error:

Filing a WER application report for exception: System.Data.SqlClient.SqlException (0x80131904): Violation of PRIMARY KEY constraint []. Cannot insert duplicate key in object 'dbo.#processingItems'.

How can I troubleshoot this further?
